    exercise harder :) I use to write the new words on paper so I can exercize even the writing :D
    write a lot and repeat all the words you remember everytime you can :)

    I redo the chapter tests over and over and over and over and over and over and over and over and over and over and over.... Until I have them mastered, before moving on the the next one. Then I do the 2 chapter tests together as a single test session, and do that over and over and over.... It would explain how I got all the points I have :D

    think less about it, let your mind relax, unconsciously you can make up your own mnemonic associations with the kanji to help you remember. don't feel stuck when you get things wrong, just keep going, and before long you'll notice that things that used to frustrate you (kanji that look the same at first, words that sounded the same) will differentiate with more clarity in your mind.
    sounding out the words is always good. then go watch anime or japanese TV subtitled, and you will notice that you recognize words you learned when hearing them in the anime/TV. and hearing them in a sentence will help you notice grammar associations with the words.

    Check out Memrise. It's like Anki - in that it is based on an SRS algorithm - except it marks you instead of asking you to mark yourself. The end result being that you get quizzed, you can't "well, I KNEW that even though I chose the wrong answer", so you reinforce items you learn.
