This happens from time to time. People have posted about this issue very frequently for years. I think the cause is that communications broke down between the user's browser and the website server at some point between the question having loaded and its answer being marked.
When this occurs you shouldn't lose any XP or your combo. If you look at the top where it says "Answer wrong: 27 EXP", if it was really deducting XP it would have a minus sign before the EXP. Also, upon clicking an answer, I think it would appear in your encounter history immediately too.
It's just that the default action that seems to happen when the page no longer has contact with the website server is to mark the selected answer in red, even though it can't find out anymore which answers are correct and incorrect, and doesn't send your answers to the server or effect your score.
So there shouldn't be anything to worry about at all when this happens. But it is misleading to the user, making it look like the answer they chose was wrong. It really would be good if this could be changed to appear less confusing to the user (such as an error message saying to refesh the page instead). ^^
