I think the problem is that the "wrong" correct answer was the first one in the list, meaning after seeing this you wouldn't normally feel the need to read any of the other three answers. And to make matters worse, the "correct" correct answer was at the bottom of the list, as far away from the other one as possible. So the chances of noticing there were two potentially correct answers was pretty slim.
This is the kind of question where my combo comes to an end with my typing mod. Since the answer choices list is hidden with my mod, I would have no idea that there were two potentially correct answers in the list and would just type the first word I that comes to my mind for meaning "happy". Probably a 50:50 chance of whether I'd think of うれしい or しあわせ first. XD

Hmm, I think the difference is that 幸せ is the noun version of "happy" while 優しい is the adjective one. I could be wrong though but if not then it should be marked next to the question.
