In 1988, 19 yr old Boeing 737-297 serving Hilo & Honolulu in Hawaii 20 min after takeoff part of the roof ruptured at 24,000 ft. Of the 89 passengers and 6 crewman there was one fatality; a flight attendant for 37 yrs was sucked out of plane, her body never found. It was determined that the accident was caused by metal fatigue exacerbated by crevice corrosion. The plane was 19 yrs old and operated in coastal environments consisting of salt & humidity.
